out on-line and E-mail to us or downloadable forms
in “Word” and “WordPerfect” that
you can save to your computer for future use.

These forms will advise you of the data required
in order to prepare the appropriate documents for
you and also our fees to provide the service. If
you are not clear on the fees you may click on “Fee
Schedule” for a full delineation of the
fees for our different services.
If you E-mail the forms to us, we will begin processing
your matter upon receipt of the requested fee and
hard copy of the additional data requested on the
data form, i.e. Summary Plan Description, Judgment
of Divorce, Settlement Agreement, etc.
For the QDROs there are two separate sets of forms.
One set is for active employees and one set is
for employees who are retired and collecting pension
benefits.
